Arteriovenous Fistula coding - AVF

what CPT® code(s) do you use for basilic vein transpostion with elevation/superficialized performed at same operative session instead of a two-staged surgery ?
CCI edits say 36819 is column two code for 36832, but 36819 is the primary surgery. Any other suggestions?

Or should additional reimbursement be requested from carrier for 36819 with a revision and elevation in operative session?
