999 rejection

I am trying to understand and find the problem why my batch to Blue Cross/Medicare denied due to : 999 and validation report rejection. I just found out that segments "k2 and K3" is where the problems are. I don't know what those means. I have called the software vendor twice with no response yet. Help!
