Vascular coding

The vascular physician that I do billing for coded his surgery as CPT® codes
34832, 34805, and 36200. We were paid for CPT® code 34832, but the other two codes were rejected as needing a qualifying procedure.
Can these codes be billed together? If so, what qualifying procedure do they need?

34805 and 36200 are both bundled to 34832 and no modifier is allowed. The only code you can be reimbursed for is 34832
