Specialist Visit Question

We are a specialist's Office, I am trying to find out if we could bill a 99212 and a 99396 with the 25 modifier together? I know you can for a family Dr, but what about a specialist?

She is a PM&R Dr., no she is not doing a preventive medicine.

Since she is a PM&R Dr., would fall prevention be considered preventive?

I don't think a Physical Rehab physician would be performing or meeting the components of a 99396. It would be highly unusual.
