Dissection with mediport placement

Dr. did a left axillary node dissection/subclavian mediport placement. I coded it as:
36561
77001-26
38525.
Patient had partial mastectomy just 68 days ago so she is in a post-op period.
My question is this, would I use modifier 79 on 36561 as it is unrelated to procedure during post op period. Then modifier 78 on 38525 as unplanned return to op/procedure room by same physician following initial procedure for related procedure during the post-op period?
