CPT® 89331 - Retrograde Semen Analysis

Does anyone know what ICD-9 would work with CPT® 89331? I have tried everything from 792.2, V26.21 and 608.87 and Bluecross continuously states that it doesnt meet their medical technology assesment criteria. Very annoying. Thanks.
