ROS when pt can't respond

I remember from class that, if the pt is unresponsive, dementia etc, and is unable to answer provider questions about the ROS, that the provider can still get full credit if they document an attempt was made but not completed because of patient's condition. Is this information still true and, where might I find it in the guidelines?
