Therapeutic vs Diagnostic G0447 Intensive Behavioral Therapy for Obesity
Clarification that is the new code G0447 , (not G0477 )

Hello,
Per Medicare Benefit Policy Manual, Pub. 100-02, Chapter 6, Section 20.5.2 and 42 CFR 410.27 and per your previous article
"Supervision Requirements for Diagnostic Tests"

Is the new CMS G0477 Intensive Behavioral Therapy for Obesity-required to be performed under General, Direct or Personal Supervision by a Physician or Non Physician (PA,ARNP) practioner? ( Personal Supervision - means a physician must be in attendance in the room during the performance of the procedure)

Could you confirm this is a Therapeutic vs a Diagnostic service in Medicares intent?
Where face to face is spelled out in the NCD -does that supersede the Medicare Physician Fee Schedule?
Per the link you gave in your supervision article I believe the supervision indicator is an 09? What is the definition of 09??

Medicare beneficiaries with obesity, defined as Body Mass Index (BMI) equal to or greater than 30 kg/m. In 2003, the USPSTF found good evidence that BMI 'is reliable and valid for identifying adults at increased risk for mortality and morbidity due to overweight and obesity.' The USPSTF also found fair to good evidence that high intensity counseling combined with behavioral interventions in obese adults (as defined by a BMI = 30 kg/m2) 'produces modest, sustained weight loss.'
Medicare coinsurance and Part B deductible are waived for this service.
Counseling is furnished by a qualified primary care physician or other primary care practitioner in a primary care setting (supervision rules do not apply (9)).
IBT for obesity consists of the following:
Screening for obesity in adults using measurement of BMI calculated by dividing weight in kilograms by the square of height in meters (expressed kg/m2);
Dietary (nutritional) assessment;
Intensive behavioral counseling and behavioral therapy to promote sustained weight loss through high intensity interventions on diet and exercise.
Effective for services on or after November 29, 2011, Medicare will pay claims for G0447, only when services are submitted by the following provider specialty types found on the provider’s Medicare enrollment record:
01 - General Practice
08 - Family Practice
11 - Internal Medicine
16 - Obstetrics/Gynecology
37 - Pediatric Medicine
38 - Geriatric Medicine
50 - Nurse Practitioner
89 - Certified Clinical Nurse Specialist
97 - Physician Assistant
If your specialty type is not one of the above, your claim will be denied
