Use of Modifier SA on the E&M for visits billed by an APRN

Can you give me any background on the modifier SA that is not in the CPT® book. Have been informed recently that we are to attach modifier SA to the E&M if billing under an APRN to Medicaid. Have never heard of an "SA"
