Can you bill for a hospital discharge and nursing home admit on the same day?
By the same token could you also bill nursing home discharge and hospital admit on the same day?

When the patient is admitted to the hospital as an inpatient in the course of an encounter in another site of service (e.g., hospital emergency department, observation status in a hospital, physician's office, nursing facility) all evaluation and management (E/M) services provided by that physician in conjunction with that admission are considered part of the initial hospital care when performed on the same date as the admission.
Exception CMS: You may report a hospital discharge-day management service on the same day as an admission service. Contractors pay the hospital discharge-day management code (99238-99239) in addition to a nursing facility admission code (99304-99306) when they are billed by the same physician or group with the same date of service.
