colonscopy screening vs diagnostic

in procedure report, what would you documentas the Procedure and the Pre-operative Diagnosis for patient who presented for scheduled screening colonoscopy but just prior to scope reports history of diarrhea (intermittent)?

If patient meets the qualifications for a screening then I would use the screening code with V76.51 or a high risk screening code and the symptom as a secondary diagnosis.
