confusion due to hepatic cirrhosis

I am still new at this and have an error dx as confusion due to hepatic cirrhosis. Would I could this as confusion and then hepatic cirrhosis or would I code Hepatic Encephalopathy. Any help would be appreciated.

Hepatic encephalopathy is a syndrome observed in patients with cirrhosis. Hepatic encephalopathy is defined as a spectrum of neuropsychiatric abnormalities in patients with liver dysfunction, after exclusion of other known brain disease. Hepatic encephalopathy is characterized by personality changes, intellectual impairment, and a depressed level of consciousness. An important prerequisite for the syndrome is diversion of portal blood into the systemic circulation through portosystemic collateral vessels.
I would not assume this is the condition unless the physician documents it. I would code the hepatic cirrhosis followed by code for confusion, given your statement.
