Chemotherapy additional hours

How do you handle additional hours when they are from more than 1 chemotherapeutic, eg. Taxotere x 2 hr and Herceptin x 2 hours? Codes would be 96413, 96415, j9171, 96417, j9355, but should the 96415 get billed x 2 with both J-codes?

To report the 1st hour of Taxotere, 96413 (1 unit); then code 96415 (1 unit). For the Herceptin, 96417 (1 unit), and and 96415 (1 unit). Your question is, should 96415 get billed twice? The answer is you can bill 96415 with units of 2. You bill the appropriate J code for the Taxotere (ex, J9171 docetaxel, injection, per 1 mg), bill units for dose administered more than 1 mg. Bill Herceptin J9355, 10mg. If more than 10 mg, bill multiple units.
