Lipoprotein (a)

I had a doctor ask if insurances are reimbursing for Lipoprotein (a). CPT 83695.This doctor wants to possibly add this test to his test menu for his in-house laboratory. I see it on the Medicare fee schedule, but does anyone know if a LCD exists to regulate this test? What ICD9 codes are accepted? Any limitations?
