CPT® 80801

We are a substance abuse facility and as of July 2012 BCBS of FL has been denying our claims for inproper billing. We billed CPT® 80101 with rev code 0300. What is the best way to bill CPT® code 80101 Urine testing to BCBS? and what is the Revenue code that we should be used? We've been told Rev code 0300 is invalid.
