denial of cpt code 12032

We have received two separate denials for payment on 12032 billed with 11402 by United Healthcare. They have stated that this must be formally appeal with Ingenix, their coding software company. I am in need of supporting documentation other than the AMA's CPT code book definitions of when can be used. I have already submitted that in my first appeal with no success.
