diagnosis denial

we billed CPT® codes 64635 w/LT modifier and diagnosis codes M47.816 and M47.817 and on the same claim we billed 64636 w/LT modifier and the same diagnosis codes as with the 64635. The insurance company has denied stating invalid diagnosis. Any suggestions on this one? Thank you!!!
