TCM - Non-face-to-face Contact
I understand that TCM requires, "Communication (direct contact, telephone, electronic) with the patient and/or caregiver within 2 business days of discharge." I was recently asked whether a hospital representative can perform this function on behalf of an independent practice as part of a community based care management program. The hospital representative would provide the practice with the documentation evidencing the contact. Would this be sufficient in billing TCM or must the practice perform this function either through one of its own employees or a contracted employee?
