Modifiers

Can someone please tell me how to use these modifiers correctly 25, 59, 76

example: basic e/m visit, dr wants patient to have a bone density exam.-Do you use a modifier for this.

example: basic e/m visit , dr. Performs an ekg and gives patient an injection and then wants a echo cardiogram performed all in the same day???

please help
