Shared/split services

I have a question about shared visits between and MD and ARNP. The pt presented for an annual physical with the MD. MD did the history, part of the exam, and the assessment and plan, but due to her broken arm, she called in the ARNP to perform the pap/pelvic. Not a Medicare pt. The ARNP signed under the exam and the MD signed at the end of the note stating that the ARNP performed the pap/pelvic exam. Who should this be billed under, MD or ARNP? Thanks for your help.
