charges for discontinuing 5fu cad pumps

We just recently went to EPIC and ever since the transition we have had billing issues. Is discontinuing a 5fu CAD pump after a 46 hour infusion a statistical visit or is this a low level established visit? I have read some insurer's consider this part of the initial treatment while other insurer's allow a visit? Which is correct and can you give me codes? We have changed it so many times now we are going crazy.
