Bundling 95004 and 95024

I have an insurance company that is trying to deny us payment for both 95024 and 95004 stating that per The American Medical Association of The American college of Medicine that 95024 is considered integral to procedure 95004 and is therefore not compensable. I can not find this documentation anywhere and we have had no problems with other, much larger, insurance companies.

Does anyone know if this documentation exists, or better yet, documentation to the contrary? I would appreciate any help that I can get!
