E&M code with lesion removal

when a patient comes in for the physician to check on a lesion and the physician removes it in the same encounter, can I charge an office visit as well? The patient was not seen for any other reason.

When did he decide to remove it?

If he knew he was going to remove it at the previous visit then no you could not, but if at the time of visit he and the patient decided to remove it after they reviewed then yes.
