Reimburse for code DX 49560

What is the standard insurance reimburse for code DX 49560 ? I'm filing my reimburse for employer and the receipt is not line item (had 2 procedures).

Do you mean for procedure code 49560? Technically, there is no 'standard reimbursement' for any procedure code because it is dependent upon the insurance company. I would look to the Medicare fee schedule for your locality to gauge what range the reimbursement should fall into.
