Flexible Laryngoscopy

Is this correct? To be able to also bill for an E/M office visit my understanding is a Flexible laryngoscopy is only separately billable if the patient is a child or has undergone a failed mirror exam? I'm not able to locate such policy on CMS website. Any information and or resource you have would be appreciated. My physicians stated they have never do mirror exam with this procedure. Please advise.
