new pt "vs" estab pt

I work in a multi practice office, familypractice,obg,intmed,peds and we have a urgent care starting at 5:00pm and weekends. My question is if a new pt comes into urgent care and is charged a "new pt" code, then at a later date goes to another dr in our facility for a new pt exam can they also charge a "new pt visit"....or if the pt is seen by and intmed dr for a "new pt visit", can they than at a later date see a family practice dr as a "new pt" or vice versa between "the different specialities". Sorry if confusing, Thanks
ps. we do use the same "NPI" number

By CPT® definition, a new patient is “one who has not received any professional services from the physician, or another physician of the same specialty who belongs to the same group practice, within the past three years.” By contrast, an established patient has received professional services from the physician or another physician in the same group and the same specialty within the prior three years.

I do understand, however it states physician or another pyhsician of the ""same specialty"" to the same group, so I guess what throws me here is the ("same specialty" phrase), we do have multi drs also different "specialties". so even if a pt is seen in family practice then goes to obg, is that still considered "est pt" or family practice to int med?? sorry

Family practice is separate specialty from an internist. oB is separate specialty.
