Coding for observed track marks associated with IV drug use

I have a patient that was seen in the ED for anxiety reaction, the physician along with the nurse state " patient appears to have track marks on both arms and hands even though denying drug use". This is noted on the exam portion of the record so my question is can I code the drug use? even though the patient denies it?
