CPT® 99509 denial.

CPT® 99509 submitted to Medicare with POS 12 on CMS 1500 form and Medicare denied it as "please submit this claim on the UB-04 form" but we are billing for professional services. Please help. Thank you!

Only as of 2013 does this go on a 1500. Also look at any required mods such as HB.

It's a non-covered code by Medicare but we need Medicare denial letter of non-cover to get pay from secondary insurance. Medicare denied it as submit on UB04 which didn't work for us.
