It depends on the carrier and your agreement with the carrier. Typically you can not bill a patient if a service is deemed not covered by a government contract such as Medicare unless you have notified the patient that charges would not be covered and they have signed a waiver prior to the services being provided. The EOB will normally tell you patient responsibility and what you can bill the patient for.
