Is there any documentation anywhere that says Insurance should pay for a
prophalctic mastectomy if a pt has cancer on one side and chooses to have the
other breast removed prophalctically.

At this time, there is no federal law requiring insurance providers to cover prophylactic mastectomy. Coverage varies state to state.
