Coding for Diagnostic Blood Draws,etc

I'm having a difficult time determining exactly how to code the following (these are related to a Glucagon Stim Test with Arginine sent out to an outside lab)

Are these appropriate:
3600x1 IV Start
A4354x1 Insert Tray
96372-59 IM Injection

Here's the rub: Do I use 36415 venipuncture code or 36592 collection from established cath. If I use 36592, it states that this CPT code cannot be used in conjunction with any other code; or, would the diagnostic blood draws be bundled into another code?

Can someone with experience in this field of coding give me advice? Also, am I missing anything that should also be coded?
