CPT® code 92504

If the doctor does 92504 on both ears i bill separetely and do a 59LT and a 59RT on each line but if the doctor does only one ear should i bill with no modifiers or just do a LT or RT? Please let me know what you think. Thanks.

CPT® 92504 says: Binocular microscopy (separate diagnostic procedure). The code is reported once per claim; there is no laterality (right vs left) to this code.
