Transition of Care Appointments

We have a patient who was admitted to the hospital for a period of 5 days. Upon his discharge our Nurse was in telephone contact with the patient within 2 days of discharge and a TOC appointment was scheduled within 7 days of his discharge.

The day before his TOC appointment, he was readmitted to the hospital as an Observation Patient (24-48 hrs). On the day of his discharge he was seen in the office by one of our Physicians for a pre-operative exam. Per the Physician, this was a TOC & Medical Clearance appointment.

Technically is this still a TOC appointment since the patient was seen in the office on the same day as his hospital discharge?
