CPT code for Colon hydrotherapy

Colon hydrotherapy is the introduction of filtered and temperature regulated water into the colon. The waste is softened and loosened, resulting in evacuation. Our medical facility uses FDA approved equiptment and the staff are all licensed and certified colon hydrotherapist.

I am trying to find a CPT code for this procedure. The medical director writes orders for the colon hydrotherapy procedure and we need the correct CPT code.

Here are some considerations:

Therapeutic Enema is a reduction technique that would be billed with 74283 is primarily used to induce defecation.

74283 - Therapeutic enema, contrast or air, for reduction of intussusception or other intraluminal obstruction (eg, meconium ileus)

I’m not sure if this is applicable however there is another procedure for colonic irrigation this is a procedure used to wash out and not intended to induce defecation. This is from a UnitedHealth Care Policy, they are saying to report with an unlisted procedure code for the colon using
COLONIC IRRIGATION (NCD 100.7)

45399 - Unlisted procedure, colon

That is not to say you cannot use the unlisted procedure for colon hydrotherapy, just be sure to attach justification for the procedure.

Other codes to consider:

45915 - Removal of fecal impaction or foreign body (separate procedure) under anesthesia

97139 - Unlisted therapeutic procedure (specify)

The reason for the encounter may determine payment, if it is medically necessary, it may be covered if it is just a cleanse, it may not be covered. Be sure to verify with the patient’s insurance and have a waiver on file if it is not a covered benefit.
