Ppd skin test
If a patient comes in for PPD Skin test ONLY and the nurse administers the skin test, is an E/M of 99211 needed or should 86580 & 90471 cover the visit?

We only bill the 86580. You do not need the 90471. The 99211 depends on if the nurse does any more than place the test. If they only place the test then no.
