Is this acceptable to use?

The majority of this 45 minutes visit was spent counseling the patient on treatment options.

re: time based billing

if memory serves, what we use is more exacting:
. . . that "50% or more of the session was spent counseling the Clt/Pt on: the risks, benefits, and potential ASEs of the medications recommended" . . . or whatever other management counseling was provided. We were told that if this type of detail was not provided (when time-based billing was used) the note would not be acceptable for billing purposes.
