puncture wound

Patient had a 4mm puncture wound on rt foot at base of rt toe. Doctor removed the flap, cleansed, and removed a 1mm piece of glass. How do I code this? So many cpt's contradict each other. We are urgent care and I will code 99204 plus j2001 lidocaine
