Esophageal banding

If two banding kits are used, how can I bill and get extra reimbursement. What about billing with a 22 modifier on 43244.

I think that is the only way to do it. However, it helps for the dr to include in the note the percentage of time longer than normal that this procedure took: i.e. "because of banding x number, procedure did take 20% longer than normal to complete, and then raise the price of the procedure by whatever percentage dr used. Send note with claim highlighting this information.
