96732 and 99211

Looking for clarification on when to bill the admin code for an injection and/or a 99211.

Scenario 1: Patient comes in regularly for migraine shot, sees only the nurse. I would bill admin code and J codes only. Is this correct?

Scenario 2: Patient brings in on meds; i.e. B12, testosterone. Someone told me in this situation bill the nurses visit only, 99211.

Scenario 3: Patient was seen by the doctor on Day 1 and diagnosed with URI and Rocephin was ordered x 3. On days 2 and 3, patient does not see the doctor. Should I bill 96732 or 99211?
