CPT® 64450

Hi,

We are a Podiatry by specialty and we are billing CPT® 64450 with Modifiers LT and RT in two different lines to Medicare. The service is performed mulitple times by the provider based on the medical necesscity, however Medicare of Florida denies the service stating "Not covered when performed during the same session/date as a previously processed service for the patient and it has a remark code CO 236 indicating " This Procedure or procedure/modifier combination is not compatible with another procedure or procedure/modifier combination provided on the same day according to the national correct coding initiative."

We realized that these claims are being filed with CPT® 11720, Hence I would need to know whether we are lacking a modifier or not full filling NCCI guide lines. Your valuable suggestions would be appreciated.

Bilateral procedures bill charges only on one line with mod50, 1 unit.
