Is it ok to perform an admitt h&p everytime a pt. Is readmitted
To the facility? Thank you!

Yes, as long as it is medically necessary.

If the patient was in the hospital for only a few days, and returned without too many changes, a subsequent visit might be more appropriate. If the patient left the SNF due to a stroke, say, and returns with a new treatment plan/new medications, etc, it would of course be medically necessary.

Most situations would require a new admission, but if you have a lot of bouncing back and forth, think about a subsequent visit.
