Port flush

What code what I use for CVAD maintenance. The diagnosis states "poor venous access" and that's all the patient received.

Not sure what that means because documentation should explain the service rendered. For irrigation of a VAD but you may want to look at 96523 which can only be billed if it is the only service performed, in the office setting.

That is the CPT® (96523) we used but we need an ICD-9 for the reason we performed the port flush.

catheter diagnosis

Why is the catheter in place example for dialysis: V56.1 Fitting and adjustment of dialysis (extracorporeal;)
(peritoneal) catheter;
Removal or replacement of catheter;
Toilet or cleansing.
Commercial payers will have their own policies for port flushes. APCs have their bundled or packaged rules. As they say, until we know the whole story, it's difficult to be accurate.

Patients have the CVAD for chemotherapy. I read in another post to use v58.81 and the diagnosis code (162.9 for example).
