Vaginal lesion codes

Hello!!

I have had an ongoing struggle trying to find codes when patients have genital lesions. What ICD-9 codes should I use for vulvar lesion and vaginal lesion. Also, what ICD-9 code would be approptiate for skin tags on the genital area and breast area that are being irritated by an undergarment. I find that some insurances will not pay due to these being cosmetic.

skin lesions

if a biopsy is taken and microscopically examined, due to signs/symptoms, the results of the tissue examination should be the diagnosis for removal (if done).

If a lesion poses no threat to health or function and is done for cosmetic reasons, would be considered non-covered. If the patient requests a filing to Medicare for denial purposes apply modifier GY to appropriate procedure code with a diagnosis of V50.1.

Benign skin lesions, example skin tags, that pose no threat to health or function are considered cosmetic and non-covered when removed.
