Valid Modifier required

Hi,

Have an issue with a CPT® 87804 billed twice on a with the below mentioned DX and modifier. We are receiving a rejection as need valid modifier

87804,QW

DX R509 & J111

87804,QW,59

DX R509

Please guide me on the issue

Back in the day when I billed Medicare for pathology they wanted a 76 or 77 modifier depending if the second test was same provider=76 or different provider=77

The MUE is 3. I would try billing like this 87804-QW x 2.
