CPT® j3420 exclusion in Medicare from 10/12/2009.

so which code can we bill instead of this?

We use J3420 and get paid for it. It is still on the Medicare fee schedule.

In tx, can you still use to bill b12 the codes 96372 & j3420 ? Can you bill an e/m together with these codes? Do you use mod 59 on 96372 or you have to use 25 on e/m code?
