does anyone know if this is billed alone or can it be billed with e/m?would you use e/m with modifier-25?any problems getting paid for it from commercial payors?

Can be done alone but probably usually billed with other svcs. In a perfect world yes, the mod25 would go on the E/M visit and both would get reimbursed. Knowing CI payers I'd say it's tough getting paid for both. Some payers may not care about the mod25 and pay the visit regardless, while denying 99420. I would use mod59 on 99420 if it's distinct from a visit code that it may be inclusive to. Depends on the payer if they reimburse it and/or will recognize the mod59 to override the edit.
