Insurance did not pay for reduction of tibial plateau stress fx, can they still bundle follow up charges with the surgery?

My first question would be why did the claim deny? Can you appeal? Does something need changed? #2, if you're doing f/u visits then you're not charging E/Ms since the patient is in postop and xrays are not included in the global package.
