G codes for speech therapy

Should there be G codes attached to the 2013 valid CPT® code of 92506? I have a old claim to Medicare that keeps denying for the 92506 which is valid for 2013 (date of claim) and I cannot get it to process. I am wondering if G codes should be attached to claim. Any help is appreciated. Thank you.

It depends on the date of service. The functional limitation G-code requirement edits were activated on July 1, 2013. If the date of service for your claim was before that, the problem is likely something else. Usually Medicare will accept our therapy claims for processing even without the G-codes (of course the claims are denied) so my suspicion would be that it's a different issue.
