I am trying to order a nuclear stress test on a Medicare patient. Our ordering system keeps rejecting my order saying my diagnosis does not meet medical necessity for the test. I'm using 785.1 (palpitation), 728.87 (general muscle weakness) and 785.0 (tachycardia).

The doctor said that during the patient's colonoscopy, which was under general sedation, the heart monitor displayed an irregular beat. The patient had complained of the weakness and palpitation at his office visit, but no other symptoms.

Any suggestions on which diagnoses might work?
