MUE limit on Botox claims

I have tried several times to get the information on Botox limitation, as of now there is a limitation for Botox on number of units however it has not been published by Medicare, i would like to have someone provide an update to me ?

I would agree that four hours is consdiered a reaonable time limit during which the treated areas should not be manipulated and avoidance of vigorous physical activity or excercise.

Coding

Coding : 275 units of Dysport are administered from a 300-unit vial. Bill with HCPCS code J3490 or J0586, depending on the Date(s) of Service (DOS), enter “1” in the QB field (equates to the number of vials used) and enter “Dysport 275 U” in the comment field (e.g., Item 19).

do we know the limitation on frequncy for botox and myobloc with Medicare, this change is effective from 10/01/2010
